Form File Name

Enter a name for the custom form layout. You can use up to eight characters for the layout name.

Description

Enter a description of the custom form layout you've created.

Cancel button

Click this button to close the Save As window. The custom form layout won't be saved.

OK button

Click this button to save the custom form layout and close the Save As window.

After you click this button, the name of the custom form layout will appear in the Selected Form list that appears at the bottom of the Forms Selection window for the form you've customized.
