This entry is required for eFiling. If you registered for eFiling under your company's EIN, this is the Federal Taxpayer ID number from the Company Information window. If you entered a different EIN when you registered, that number will appear in this field.

Company name

This entry is required for eFiling. If you used your company's name when you registered for eFiling, this is the Company Name that appears in the Company Information window. If you entered a different company name when you registered, that name will appear in this field.

Address

This entry is required for eFiling. The company address that you entered when you enrolled in eFiling.

City

This entry is required for eFiling. The city that you entered for your company when you enrolled in eFiling.

State

This entry is required for eFiling. The state that you entered for your company when you enrolled in eFiling.

Zip Code

This entry is required for eFiling. The ZIP Code that you entered for your company when you enrolled in eFiling.

Contact name

This entry is required for eFiling. The name of the contact person you entered when you enrolled in eFiling.

Contact title

This entry is required for eFiling. The job title of the contact person whose name you entered when you enrolled in eFiling.

Contact phone

This entry is required for eFiling. The telephone number of the contact person whose name you entered when you enrolled in eFiling.

Contact fax

The fax number of the contact person whose name you entered when you enrolled in eFiling.

Contact email

This entry is required for eFiling. The email address of the contact person whose name you entered when you enrolled in eFiling.
