This field contains the name and address of your company. This information is taken from the Company Information window. If you want to make changes, click the arrow to open the Company Information window.

Form

Choose the IRS form, 1099 or 1096, you want to print.

Employer ID#

This field contains the Federal Tax ID number you entered in the Company Information window.

Name of Person to Contact

Enter the name of the contact person in your company that will appear on the 1096 form.

This field appears only if you choose 1096 in the Form field.

Telephone Number

Enter the telephone number of the contact person in your company that will appear on the 1096 form.

This field appears only if you chose 1096 in the Form field.

Fax Number

Enter the fax number of the contact person in your company that will appear on the 1096 form.

This field appears only if you chose 1096 in the Form field.

Email Address

Enter the email address of the contact person in your company that will appear on the 1096 form.
