This field contains the Federal Tax ID number you entered in the Company Information window.

Name of Person to Contact

Enter the name of the contact person you want to appear on Form 1096.

Telephone Number

Enter the telephone number of the contact person.

Fax Number

Enter the fax number of the contact person.

Email Address

Enter the email address of the contact person.
