Name & Address

This field displays the name and address of your business as it's entered in the Company Information window. To view or change this information in the Company Information window, click the zoom arrow.

Federal Tax ID

This field displays your business's employer identification number (EIN) as it's entered in the Federal Tax ID field of the Company Information window. If you have registered your Payroll Tax Forms program for use by more than one company, this field will provide a drop-down menu where you can select the correct Federal Tax ID to appear on the forms that you want to prepare.

Form Selection area

This section of the window displays lists of forms that you can select to prepare using the MYOB Payroll Forms Service.

Search by

This field displays the categories of forms that you can display and prepare using the MYOB Payroll Forms Service. Select Federal to display the list of Federal forms that you can prepare. Select State to display the list of state forms. If you select State, a second drop-down list will appear to the right. Use this list to select the state for which you want to display forms.

Form

This field lists the forms available for preparation using the MYOB Payroll Forms Service. The forms that appear in this list are determined by your selections in the Search by field.

Select the form that you need by clicking on its name.

Display Options area

This section of the window displays information about the time period covered by the selected report, and allows you to further define the reporting period for certain reports.

Reporting Period

This field displays the reporting period covered by the selected form. Form periods can be quarterly, monthly or daily, or they can cover a range of dates that you specify ("Date Range").

Year

This field displays the reporting year for the selected form.

Reporting Period selection list

This field lists selections for the form reporting period shown in the Reporting Period field.

• One Quarter selections (drop-down list): 1st Quarter, 2nd Quarter, 3rd Quarter and 4th Quarter.

• One Month selections (drop-down list): January through December.

Note:  Data must be calculated on a monthly basis for some forms

• Daily selections (date entry field): Enter any date in the format DD/MM/YYYY. (Default is today's date)

• Date Range selections (two date entry fields): Specify the appropriate date range for the reporting period, using the Start Date and End Date fields.

Saved (Forms) button

Click this button to open the Open Form window, where you can select a saved version of the form you need.

Register button

Click this button to open the Payroll Tax Forms Registration window, where you can enter your MYOB Payroll Forms registration code.

Display button

Click this button to begin work on a new copy of the form you selected.
