Company Information
Information in this section is derived from your entries in the Company Information window unless you change the entries in these fields. To learn more about the information that appears in these fields, refer to Data fields on Form W-2 and Data fields on Form W-3. You'll find complete instructions for completing Forms W-2 and W-3 at the Internal Revenue Service website. If you still have questions about filing payroll tax forms, consult your accountant.
Company name
Enter the company name that you want to appear on all of your W-2s and the corresponding W-3.
Trade name
The trade name (if any) that you want to appear on all of your W-2s and the corresponding W-3. If you do business under a DBA ("Doing Business As") or T/A ("Trading As") identity, that identity should appear in this field.
Address line 1
The street number and name of the business location to appear on the currently displayed W-2 forms.
Address line 2
The second line in the address of the business location to appear on the currently displayed W-2 forms. (This entry is usually a Post Office box, apartment or suite number.)
City
The city in which your business is located.
State
The state in which your business is located.
Zip code
The ZIP code for the city and state where your business is located.
Federal EIN
The Federal Employer Identification Number (EIN) for your business.
Please Note: This number is derived from your entry in the Company Information window, and must be shown on all W-2 and W-3 forms that you file. If the EIN shown in this field is incorrect, be sure to change the entry in the Company Information window, too.
Contact name
The name of the contact person entered in the Set Up W-3 Form window.
Telephone #
The telephone number for the contact person entered in the Set Up W-3 Form window.
Fax #
The fax number for the contact person entered in the Set Up W-3 Form window.
The email address of the contact person entered in the Set Up W-3 Form window.
State Information
This area of the window displays information about the states for which tax information will appear on your employees' W-2s.
State
A list of the two-letter postal abbreviations for each state for which tax information will appear on your employees' W-2s. This information is based on your entries in the General Payroll Information window and in the Card Information window - Payroll Details view - Personal Details for each employee, and can be edited for use on the currently displayed W-2s and their corresponding Form W-3.
State ID #
The state-provided identification number for your company in each state or which tax information will appear on your employees' W-2s. This information is derived from your entries in the Company Information window, and can be edited for use on the currently displayed W-2s and the corresponding Form W-3.
New button
Click New to add a new state to the State Information list. If you add a state, it will be used to prepare data on the currently displayed W-2s, but will not change the information in your MYOB Premier Accounting company file.
Edit button
Click Edit to change the ID number shown for any state. Any changes you make here will not change the information in your MYOB Premier Accounting company file.
Locality Information
This area of the window displays information about the localities for which tax information will appear on your employees' W-2s. If you need to report tax information for specific locations on your W-2s, you must identify the localities here.
Locality
A list of the localities for which tax information will appear on your employees' W-2s.
Type
The Tax Type Code for the locality shown on each line. Choices are:
State
The state where the locality is located.
New button
Click New to add a new locality to the Locality Information list.
Edit button
Highlight a line in the Locality Information list and click Edit to change the Tax type Code or State for that locality.
Termination checkbox
This business was terminated in [tax year]
Mark this box if the business identified in this window was terminated during the tax year for which you are preparing W-2s.
Command button
OK
Click this button to apply your entries to the currently displayed W-2s and the corresponding Form W-3.
