Go to the Payroll command centre and click Prepare Payroll Tax Forms. The Select Payroll Tax Form window appears.

To select a state form, select State in the Search by field and then choose a state from the drop-down list.

Select the form you want to prepare from the Form list.

If a message appears advising that some of your company information is invalid or incomplete, go to the the Company menu and choose Company Information to enter your details.

When you’ve finished editing information on the form, click Save to save a draft version of this form.

Click Next Step. A message appears requiring you to confirm that you have completed editing the form. If you have finished preparing the form, click Agree.

Click Print to print a copy of the form for your records.

If you want to eFile the form, click eFile. Enter your eFile Username and Password in the eFile Login window that appears and then click Login. Follow the on-screen instructions to file the form.
